For students who are interested in becoming an EBMAS member, please fill out this passport application and return it to Sifu Edgar or Sifu Xuan Mai with 2 passport sized pictures and $70.  Your passport must be renewed annually for $60.

All students who wish to continue testing for their student levels after 2nd Student level must have an EBMAS passport.
